Patient eligibility — preliminary checks
Pyelonephritis — immediate ED flags
Tick any that apply. Any tick = immediate ED referral.
Acute cystitis symptoms
Tick all present. Needs ≥2 for probable acute cystitis (>90% probability if no vaginal discharge).
Immediate GP referral criteria
Tick any that apply. Any tick = refer to GP (pharmacist may still supply conservative care).
High-risk history — refer to GP with / after treatment
These warrant conservative management + GP referral. Antibiotic treatment may still be considered concurrently if appropriate and no contraindications exist.
STI risk factors & other considerations
If any STI risk factor applies, treat provisionally and refer for STI testing. Antibiotic treatment may still proceed concurrently if clinically appropriate.
